Gestational age is the major marker of neonatal survival.

According to WHO every year arround 15 million babies are born too early and 1 million die due to prematurity complications. This number is rising. Whenever a baby is born too small, birth attendants need to make timely decisions such as refering the newborn to a hospital. Without critical care, newborn survival could be neglected, as well as it’s potential for a healthy life. Most of these lives could be saved with prompt neonatal prematurity identification, but current methods remain an unsolved challenge because of disadvantages such as low accessibility and high costs of obstetric ultrasound.

Preemie Test

Home Birth

Childbirth takes place at home and the midwife intends to deliver good care. The best practice for newborn assistance relies on the gestational age, birth weight, and neonatal conditions at birth. When a small baby is born without a reliable gestational age, the Preemie-test can provide clinical decision support, considering the risk-appropriate neonatal care and transport system as well as referral for hospital assistance.

Low Complexity Facilities

 A baby is born in a low complexity facility unit of care as level-1 maternity, general hospital, primary care unit with birth assistance, low-complexity birth-centers. The birth weight is 2450grams, and the gestational age is 36 weeks of gestation based on the LMP. The woman had irregular cycles. There is no gold-standard to support pregnancy dating. In this situation, the range for gestational age is 32 to 40 weeks based on four weeks of error for clinical dating.  The Preemie-test is useful to decide procedures for immediate neonatal assistance and to evaluate the need for transference to a high-complexity-facility center. 

High Level Complex

Immediately after childbirth, a newborn with unknown or unreliable gestational age demands resuscitation with initial steps in stabilization and positive-pressure ventilation . The sequence of the team actions considers gestational age information. Even the birth occurs in the level-3 of maternal care; a reliable gestational age is a critical data in the guidelines of neonatal care.
